    The SmartPHR® A holistic, interoperable personal electronic health record automatically extracts and consolidates patient information from many doctors’ unconnected electronic health records. Patients and providers access comprehensive SmartPHR records 24X7 from web- connected and mobile devices. Patients access their records via the SmartPHR user interface. Authorized providers access patient records via the SmartEHR user interface or via their native, interoperable EHRs.

    The Smart Meaningful Use EHR™ Patient-centered, electronic health record modules enable health and human service providers to supplement or replace legacy systems. Providers select interoperability, CPOE, care plan, clipboard, disease registry, observations, meaningful-use and quality-indicator modules that suit their workflow needs and enable them to document adherence to CMS incentive requirements.

REC managers will receive on-demand access to aggregate data reports, free of HIPAA protected health information (PHI), for satisfaction of REC grant commitments. No EHR on the market offers these capabilities at any price. For SmartPHRs, SmartEHRs, and REC-administrator capabilities, the total first year per-provider cost, billable to credit cards, will be $3,000.
